Offer available to qualified Mediacom Internet customers in good standing who switch a minimum of two (2) and up to four (4) lines from an eligible previous mobile provider and activate qualifying Mediacom Mobile Unlimited Plus plans or higher.
Reward is provided as a digital Visa® Reward Card and is based on the verified remaining device payment plan balance from your previous provider, up to $500 per line and $2,000 per account. Customer must submit required documentation within the specified timeframe and complete number porting and activation to qualify.
Customer must maintain continuous, active qualifying service for 18 months. Early cancellation, plan downgrade, non-payment, or disconnection may result in a prorated repayment obligation based on the remaining commitment period.
Reward card expires 12 months after issuance, has no cash value, and is non-transferable. Offer limited to one per account and may not be combined with other promotions unless expressly stated.
